Trag'Oul was also mentioned in the Sin War books. At one point Trag'Oul was going to enter Sanctuary but was stopped. I think he mentions that he entered Sanctuary once in the past. Anyway I don't think it's on his back if he can enter Sanctuary, unless he can be in 2 places at once.
The whole 'on his back' thing is metaphorical. Its not literally on his back. Trag'Oul is just its guardian, and Sanctuary being 'on his back' is just a way of saying that he is protecting it.
