It did happen some time. Usually they had long enough fuses to get away just in case though.
That is, when they developed fuses... Before fuses, you were pretty much screwed...
I can't imagine WHY in the hell you'd want to use something like a cannon without a fuse. It's pretty much a bomb going off and you just hoping to God the cannon doesn't shatter into shrapnel.
A more realistic point of view on the whole gun thing is that a Brigand class will make its way into the game. They could have a tree dedicated to firearms: stances, unique arming methods that require a resource (kind of like enchanting in WoW) could be the focus of that tree. Sniper Stance, Swift stance, explosive ball round, ect.
The gun would fit perfectly into the world to be honest, it was add a new tier to the ranged weapons prospects of the game. You have the fast bow, the slow and accurate crossbow and the very slow but much more powerful rifle. It could be a unique weapon to the ranged class like those class specific items from LoD.
That's just an idea off the top of my head though, it'd need to be properly ironed out.
Besides they simply chose the medieval era as a point of origin for the style of the game, how the world developes is entirely optional as long as it abides by the context of the story that weaves the world of sanctuary. The different factions such as the barbarians were aware of the looming darkness, though it's true that they said the people don't believe them, these underground groups could be in full animation in regards to the ramifications of what lay ahead, especially since the curse on the Nephalem was lifted when the World Stone was destroyed. (although this might mean nothing since the curse was supposed to reduce the power of the first generation humans over each passing on to the next.)
Hrm... I haven't played D1 in a long while but there where no alchemy potions in that like explosive potions and such right? Yet somehow between D1 and D2 they were created.

D1 and D2 were one after the other though, it was just not in tristram (how ironic that they drop there when you go in D2.)
I think this conversation is pointless right now, we don't know enough to paint a picture or anything. It'd be nice if this thread could be saved or something.
I prefer the old sword and sorcery better. Guns may work for some people in WarCraft games but I for one don't like to have them in Diablo. It sounds stupid to battle big "D" with a rifle, if you ask me. But seeing how they're borrowing stuff from WoW, I don't think one can rule guns out... :-/
There were no rifles back then. Hand cannons would simply have balls instead of arrows/bolts. You could imbue the hand cannon with a topaz and boom you have something that fires electric, heavy, round projectiles. In terms of practicality, not much different from a sorceress.

I'd compare it more to a amazon's bow with said topaz. Firing either heavy hard bolts or heavy round balls, still the same flying matter of doom. And with the time frame, very plausible.

Obviously there is no correlation between Sanctuary's timeline and ours, but the game gives off a medieval vibe and people don't typically relate guns with that place in time. Not to mention that Warcraft has that covered. But I argue for too many WoW 'improvements' to play that card

As for the typical person your speaking of, it's obviously Americans, (And I take no joy in saying this, as I love my country, but...) but Americans are, for all intensive purposes, idiots, and half the time don't know how to spell the name of the teacher there banging...
