If Diablo 3 is a sequel: Cain is the real evil. Don't forget, he is a human and human is the only evil there has ever been. He tricked us and we killed his adversaries (Diablo, Mephisto, etc), now he is coming for us! OMG! We will have to kill Cain in the end.
If Diablo 3 is a prequel: Cain is alive. Or we don't know him yet. Someone else guides us, and he dies in the end, only to be replaced by the most evil creature who has ever existed: Cain.
If Diablo 3 is a MMORPG: I think it will happen in the same period as Diablo 2. But in the expansion, Cain will rise to power and in the second expansion, Cian will die (kind of like Lord British! lol)
I think that Cain should be possessed by a demon that wants to destroy the world, but gets us to rid the world of the forces that would prevent that, so....in the end we find this out and fight Cain and banish the "demon" back down to hell

Without evil we cannot possibly prevail. It is a necessity which we cannot do without. How can one be deemed good, or living a proper and just life without the presence of the dark prince. Evil lays within us all, whether flourishing or lying dormant, it is most certainly there. It is a struggle that every being must face every waking second of their lives, and without it we are nothing. I embrace the darkness, the cruelty and hatred, and most of all, the destruction of the light and the good it brings. The closer you bring these terrible things to yourself, the more you can combat them. The more you attempt to elude the evil within you the worse it will get. We are all but shadows and dust, but our actions echo endlessly through eternity, whether evil or just.
I think there will be some sort of twist. He will end up being an angel, a demon, or maybe he will be son of an angel and a human (I think that falls into the founding of one of the races)

I would like to bring to light a few choice points about this particular question.
1: Etymology of the name, who was Cain? Cain was the character in the Bible who comitted the first murder in history by slaying his brother. Coincidence? Quite probably.
2: It is quite unusual that Cain is as old as he is (basically every single character in the story eventually gets slaughtered in some manner unless they happen to be an arch-angel ;-))
3: Watching the gameplay movie, Cain seemed to be intent on discovering something down in the depths of the catacombs, despite the fact that the hero had no interest in listening to it.
Summary: Cain has been corrupted, his mind tainted (And how could it not be after battling the prime evils in his own way throughout the past 50 plus years right?) You don't see Tyrael down there gaurding him anymore? (And yes I think Tyreal is in fact good. What possible motivation would he have honestly?) At some point, it's going to turn out, Cain is evil, and it's your job to destroy his demonic possession with him along with it. But just like Lazarus, he was used as a pawn, to accomplish some mission that only his trusted position could afford him. Aside from that, I still wonder about the girl in the preview movie.

it is possible he could die, i mean with some event and he silently passes, your pissed, and that leah girl takes his place, but eventrally i think he dies.
If I was the barbarian I would say to Cain inside the dungeon "Tyrael told us that the world is about to change but I didn't expect for the rainbows...":D

Cain will live for a considerable ammount atleast, until we are well settled in this game. Probably die in act 3 or 4 of D III
