Legendary Gems:
Enforcer
Increase the damage of your pets by 15.00%.
- Increase per Rank: +0.30%.
Increase the damage of primary skills by 25.0%.
- Increase per Rank: +0.5%.
- Rank 25 bonus: Primary skills heal you for 2% of maximum health on hit.
- - +0.5% damage per Rank
Posion all enemies hit for 2000% weapon damage over 10 seconds.
- Increase per Rank: +50%.
- All enemies you poison take 10% increased damage from all sources.
50.0% chance on hit to gain Swiftness, increasing your attack Speed by 1% for 4 seconds. This effect stacks up to 15 times.
Increase per Rank: +1.0%.
Rank 25 bonus: Gain 1% Cooldown Reduction per stack of Swiftness.Bane of the Trapped
Increase damage against enemies under the effects of control-impairing effects by 15.00%.
Increase per Rank: +0.3%.
Rank 25 bonus: Gain an aura that reduces the movement speed of enemies withing 15 yards by 30%.
Any of the above are good, while Gem of Efficacious Toxin is the worst. Also a good option would be Moratorium
It seems that simplicity will not effect the DMG of fetishes, only your DMG. So best would be to take Gogok of Swiftness and Enforcer. As primary DMG source.
If you go with Hwoj Wrap and Locust Swarm instead of Gargantuan. You can take Bane of the Trapped for even more DMG and CC.
Anyway I made a video with all the detailed info like pagagons, spec etc.

It seams that legion of daggers might be a better choice than Head hunters, For some reason unknown to me they also do PSN dmg. However if you leave them to attack on their own they will do phys dmg which is much lower so it is a bit unclear what is best. All depends on what is the downtime you have on darts while walking etc.
That being said, this build is not without its drawbacks. The pet AI has been improved, but fetishes will always stop where they are and fire when you do. They won't auto-correct if they're blocked by a wall, so micromanagement of pets is a lot more intensive than the Physical Pets build. In regards to passives, I like your choices, but I've swapped Bad Medicine for Jungle Fortitude.
I think your guide might benefit from some additional recommendations for rings. I'm using double Unity, but TMF is also a fair choice for early-mid twenties. You're absolutely correct in that the Hellfire will be best in slot, but your choices of Xephirian or Julia's are great, too.
In terms of gem choices, I think Simplicity's Strength should be swapped out for Bane of the Powerful until we get definitive word on whether or not the fetish darts also are increased by 25%. Moratorium is probably a better choice than Efficacious Toxin as we a solid survivability gem.

I agree with your choices for gems. Might be better. Regarding positioning yeah you do need some micromanagement. It isn't perfect. I've made it in about 1h. So it couldn't be perfect. I'm playing seasons now, so I don't have all the required items. But I will get this updated with new info as soon as I get some more info.
and yes legion of daggers is better.... they shoot a POISON DART when you do... the more fetishes the more psn damage. anyway i like this build a lot... it really works well because you are not as squishy.. The down side is when there are hight differences in the levels or walls. As far as mobs go, electrified will kill every melee champion if they are foolish enough to walk in. And wallers are annoying. Other then that it is a fun build.
P.s. I REALLY recommend that you use the stun dart.... the 3 darts only work for you and not your fetishes and the dmg does not outweigh the benefit of a cc. Some rg's will stay stunned almost the entire time and you keep most champions away from you for quite a while.
Although I like your option about Locust and hwoj. Will be adding that to description soon.
Grasp of the dead is plain simply BAD. It may give you more dogs but those dogs are weak, will die often and have no DMG. As far as making a crowd you really don't need more than 8 + 15 fetishes. Not every poison dmg is good, unfortunately for us
I find using Haunt of Vaxo coupled with Snake to the Face (stun rune) also very useful. The clones provide a measure of DPS and help tank a little.

The biggest difference is however in the gems. I think that Bane of the powerful is insane 20% dmg and 15% dmg against elites is too good to pass on. Also the Pain enforcer and gogok of swiftness work great together as you build up IAS like crazy. I think in a large pack with all the buffs you get easily to 2.8-3.0 aps (from under 2 aps as the base).
I think some thought might still be given to the garg slot... perhaps something else could be used. leeching beasts are ok, but they die easily...

Legion is the obvious better choice because basic math dictates 8 > 7. For some reason with Carnevil they shoot poison darts...well obviously they do poison damage. If there are mobs you want to kill, you want them to be shooting anyway. If you're gonna stack physical damage for the gaps between shooting you might as well run the usual mask of jeram fetish build.
