For fifth Passive of Hellfire:
Grave Injustice [Solo]/Gruesome Feast [Group]


Gems:
Helm (Amethyst)
Chest/Legs (Topaz/Ruby[Toughness in solo play]


Head:
Mask of Jeram is required, either in cube or equiped. You can then use Andariel's or Grin Reaper.


Belt:
If you have a great witching hour equip it, otherwise use belt of transcendence as it will give you a ton of fetishes...
they won't make any damage but are a "meat shield" to mitigate some damage


Weapon:
Barber is required and its the base of the whole build. Either equip or cube it depending of what you have.
Then use Sacred Harvester or Voo Juicer.
Voo Juicer cause gives a lot of utility, healt, mana, and you can use Manitou for extra damage.
Harvester will keep you tougher and give you more Int, meaning more damage, but keep an eye on your 10 stacks
Do not use Voo in HC.


Legendary Gems:
You can swap Pain Enhancer for Bane of Stricken in solo play.
Pain Enhancer is used over Bane of the Stricken for group play because LoN is used as the "trash killer" and the other DPS is used as the "RG Killer" in most instances. Pain Enhancer allows for a tremendous attack speed bonus


Skills:
Zombie Piranhas is used as a wide range area for 15% damage increase while keeping the base 8 second cooldown.
You can switch for Piranhados in solo play for more control of the mobs.
You can also switch Big Bad Voodoo for Spirit Walk Healing Journey if you are dying too much

In party with a Healing Monk you can drop Phlebotomize for spirit is willing



Paragon:
No need for area damage - it has no effect on phantasms. Stack as much AS as possible, that's where your damage is coming from.


Try to have as many ancients as you can, to make use of LoN
