Shoulders chest and offhand *must* have max Garg dmg.
Stack crit, vit, and AD. Roll off Mana regen as long as you are using Rush passive. If you have tons of paragon points then max out mana.
Also get some LOH or you will die/need spirit vessel passive like every other dirty hippie on the leaderboard.
Swampland can be good in a party with lots of density. If you are going to be balls deep in monsters all the time then hwoj wrap will slow everything close to you and help max out the close up dmg.
Cold and physical damage make sense so choose one and build skills around it. Somewhere there is something in the patch notes about physical dmg and gargs but screw that. The devs really *reaaaally* want you to use physical but you are going cold aren't you? I thought so. Me too.
The haunt poison rune is for suckers. If you have more than 6 paragon levels don't use it. If you do try out the haunting girdle belt with it- should be good right? No. It isn't. Don't make me spray you with the hose again.
Stricken should be a good gem to use but it isn't. If you are close to the RG you are toast anyway. Esoteric is impressive with this build so use it.
Play around with different belts and don't worry about the schmucks telling you to use witching hour. They didn't take the time to type out this crappy test build, did they? Nope.

Okay don't use hwoj belt. String of ears is good though. WD is just broken and bugged but will prob be playing it a lot in S7.
I've been working on a build with swampland wader pants w/ wormwood, ROE, and using sacrifice. It's fun but too squishy so far. Meh... I'll keep working on it though.
