Itemization

There are no real required items. I've made some general suggestions above, the most notable of which is the new Undisputed Champion belt that enables all the runes for Frenzy. This makes for a significant increase in single (and multi-target) DPS and utility (stuns).

Paragon Priorities

Vitality
Maximum Resource

Offense

Critical Hit Chance
Critical Hit Damage
Cooldown Reduction

Movement speed is my preference. Otherwise Strength.

Balance Crit Change and Damage

Resist All is king early on

Area Damage is the best option for Utility early

Build Guide

Sharing the build I used leveling up to 70 and until I completed my first T6 capable set (which happened to be Raekor). I was able to zoom through T4 with this build solo or in groups.

The beauty of the build is that it provides flexibility in gear and successfully bridges the gap between pre-T6 gameplay and end game set builds by creating sustainable Fury pool (less dependent on Fury too), efficiency (good single and multi-target DPS), and mobility+speed.

I tried Whirlwind and just didn't find it to compete in terms of utility and damage output, though you could replace Furious Charge with WW if you prefer that and maintain the same concept (for example if you found a great piece or two of gear with +WW dmg on it).

Basic Skill Build: http://us.battle.net/d3/en/calculator/barbarian#WckRSU!YZhj!bcbcZc

Skill Notes: Many of the abilities can be swapped out to fit your play style and needs i reaction to the gear you find. For example. I found a sweet pair for StrongArms around LvL 60, so I started using Ground Stomp with Wrenching Smash to proc the 25% dmg increase until that not longer served me as well as other options.

If you don't have a lot of crit early on, you can use Battle Rage - Maurader's Rage instead.

Swap out the War Cry rune what makes the party and you most effectively. I generally think Impunity and Invigorate are the best options.

Change the sprint rune if you are in a party to Forced March if appropriate to keep up your group run.

Basic Gameplay: 1) Assess situation

2) Less than 4 mobs generally = Rend and then Frenzy.

3) More than 4 mobs generally = Furious Charge, Rend and repeatedly Charge thanks to Merciless Assault to spread more Rend.

4) Keep Battle Rage and War Cry up

5) Sprint to keep up the pace.

Misc Notes As a rule, try to spread Rend with Bloodbath.

Don't charge single or small groups of mobs less than 4-5. This will help ensure your Furious Charge cooldown stays up. It's a great way to escape in an emergency too.

Swap out passive skills to meet your needs.

Weaknesses: Waller packs

Progression: Fill in Raekor's pieces as you get them. Raekors 4pc is a big boost, so get the Ring of Royal Grandeur and Charge to win once you have 3pc Raekors.

Suggestions/Feedback/Constructive Criticism are welcomed!
