Full MotE along with Lut Socks allow you to fill the screen with earthquakes while whirlwind is simply a tool to keep you mobile with constant heals coming in from Bloodthirst. Health from Globes is a huge stat for this build since you'll constantly be spinning, my life per fury spent is 2,664 and along with ~70% uptime on Ignore Pain I can stand in a whole lot of shit for a long, long time without going down. I think you can reasonably get Ignore Pain uptime upwards of 80%, my Leoric's Crown is only 87% increased effectiveness and my Pride of Cassius only adds 5 seconds and I have room for improvement on one or two more rolls. My Furnace rolled LPH unfortunately but grabbing 6-10% from a weapon would be a nice boost. I run Blackthorne's for the set bonuses but I'd switch out my amulet without a second thought if I found a nice Xephirian or Cameo (unfortunately the ones I always find are crap).Pain Enhancer is a nice choice for a physical build since you can boost the damage to ~1000% per second without too much trouble just from your neck and wrist slots. Bane of the Trapped is a no brainer due to the way the damage is calculated and Moratorium just gives you that much more survivability.
The EQ/Cave-in and Leap/DFA combo keeps things nice and clumped together and stunned for a large portion of the time. I used to run WW/Blood Funnel but had to run Earthen Might to keep my fury up and I've found that 25% more EQ damage from Berserker Rage far outweighs the damage boost from using a physical-based WW. I don't find it difficult at all to maintain >95% fury, even keeping Battle Rage up. Battle Rage is interchangeable and can be swapped out for something like War Cry if more survivability is desired. WotB and Boon can be swapped out for that matter but if there is one thing the build lacks it is single-target damage and I find those help with that.
I've run several different iterations of this build (IK helm/belt/chest for perma-CotA, cold, fire) and I think this one definitely works the best, at least for how I like to play. I'm not pushing into level 40+ grifts by any means but I can run into the 30's comfortably and don't feel super squishy if I drop my Unity for group play.
